Terms of Service

By participating in our program, you agree to our Terms of Service, which outline the guidelines, responsibilities, and expectations designed to ensure a positive, fair, and transparent experience for all users. Please read these terms carefully to understand your rights, obligations, and important details about our program's operations and policies.

1. Consent for SMS Communications
Upon granting consent to receive SMS communications from our company, please be assured that your phone number and SMS consent will remain confidential. We will not sell or share your information with third parties or affiliates. Your phone number and consent will solely be used to facilitate communication with you and will not be disseminated for marketing purposes.

2. Types of SMS Communications
After obtaining consent, the following types of messages may be sent to you, depending on your interactions with our services:
  • Customers and Guests: Updates related to appointments and benefit applications.
  • Job Applicants: Notifications regarding the status of your application.
  • Appointment Reminders: Reminders about scheduled appointments or events.
  • Example Message: Hi [Customer Name], this is a reminder about your upcoming appointment with Tulsa Responds on [Date] at [Time]. We look forward to seeing you! To opt-out, reply "STOP" to unsubscribe. Message data rates may apply. Messaging frequency may vary. For assistance, reply "HELP."
Standard Messaging Disclosures
  • To opt out of SMS communications, simply reply with "STOP."
  • For assistance or inquiries, reply with "HELP."
  • Message and data rates may apply
Sample response for "HELP" reply:
"For further assistance, please visit our website at tulsaresponds.org or contact us at 918-900-0918."
 
Message Frequency
You may receive approximately 5-10 messages per day related to your requests. However, the frequency may vary depending on the nature of the communication

Opt-In Method:
You may opt-in to receive SMS messages from Tulsa Responds in the following ways:
  • Verbally, during a conversation
  • By submitting an online form
  • By filling out a paper form
Opt-Out Method:
You can opt-out of receiving SMS messages at any time. To do so, simply reply "STOP" to any SMS message you receive. Alternatively, you can contact us directly to request removal from our messaging list.
Additional Options:
  • If you do not wish to receive SMS messages, you can choose not to check the SMS consent box on our forms.

Consent for using tax return information to determine eligibility for other services

Federal law requires this consent form to be provided to you ("you" refers to each taxpayer, if more than one). Unless authorized by law, we cannot disclose, without your consent, your tax return information to third parties for purposes other than the preparation and filing of your tax return. If you consent to the disclosure of your tax return information, Federal law may not protect your tax return information from further use or distribution.

You are not required to complete this form to engage our tax return preparation services. If we obtain your signature on this form by conditioning our tax return preparation services on your consent, your consent will not be valid. If you agree to the disclosure of your tax return information, your consent is valid for the amount of time that you specify. If you do not specify the duration of your consent, your consent is valid for one year from the date of signature.

For your convenience, Tulsa Responds is prepared to provide referrals for several auxiliary free benefits and services. To determine your eligibility for these benefits and services, Tulsa Responds will need to use your tax return information.

This information includes your adjusted gross income, household size, dependents’ age(s), and zip code.

If you would like to authorize Tulsa Responds to use your tax return information to determine your eligibility for auxiliary, free benefits and services please check the corresponding box on our free tax filing appointment form. If you believe your tax return information has been disclosed or used improperly in a manner unauthorized by law or without your permission, you may contact the Treasury Inspector General for Tax Administration (TIGTA) by telephone at 1-800-366-4484, or by email at complaints@tigta.treas.gov.

Consent for disclosure of tax return information for advertising/marketing

Federal law requires this consent form to be provided to you ("you" refers to each taxpayer, if more than one). Unless authorized by law, we cannot disclose, without your consent, your tax return information to third parties for purposes other than the preparation and filing of your tax return. If you consent to the disclosure of your tax return information, Federal law may not protect your tax return information from further use or distribution.

To continue providing this free service, Tulsa Responds will need to disclose anonymous aggregated return data for marketing and/or advertising. Anonymous aggregated data such as zip codes, demographic data, and refund/credit dollar values may be disclosed. No personally identifiable information, such as your name, Social Security number, address, or other personal information will be disclosed.

If you would like to authorize Tulsa Responds to disclose your tax return information for marketing and/or advertising, please check the corresponding box on our free tax filing appointment form.
