  • Medicaid / Sooner Care Enrollment Services: paystubs, income and expense documents. 
  • Eligibility for SNAP or Other State and Federal Benefits: photo identification, income and expense documentation, disability paper work, utility bills.
  • Information that you provide by filling in forms on our Website. This includes information provided at the time of requesting services. 
  • Recordings of telephone calls with you that may be recorded for training purposes. 
  • Records and copies of your correspondence (including email addresses), if you contact us.
  • Your responses to surveys that we might ask you to complete regarding your experience.
